The Immunization History
screen enables you to enter a client’s immunizations that occurred elsewhere
before coming to your clinic. These
historical shots will appear on the Immunization Master
screen and will be a part of the client’s shot records.
Following are the fields to
be filled in:

Description |
Date |
Date the immunization was given. Date must be after the client’s birth date and before today’s date. |
CPT |
The CPT code for the
vaccine given. You can enter the CPT
code, and the corresponding vaccine name will appear in the vaccine
pull-down. If you do not know the CPT
code, you can select a vaccine with the vaccine pull-down and the
corresponding CPT code will appear. |
Vaccine |
Select from a pull-down list. |
Given By |
Free-form field for the person, clinic, doctor's office, or organization's name who gave the immunization. |
Reaction |
Indicates if the client had a reaction to immunizations given on this service date. Select from a pull-down list. The default selections will be No Reaction, Not Read, or Reaction. However, if Tuberculosis Skin Test is selected, the selections will be Negative, Not Read, or Positive. |
Comment |
Free-form comment field. |
Measure |
(Only enabled when
Tuberculosis Skin Test is selected from the Vaccine pull-down.) Numeric results of the tuberculosis skin
test in millimeters. |
Date Read |
Date the skin test results
are read and evaluated. Must be after
the service date. |
Delete? |
A checkbox indicating
whether you wish to delete the record.
If checked, the record will be deleted upon saving. |
The following buttons can be
accessed from the Immunization History screen:
Adds a new row containing
all of the above fields to be filled in.
Saves all changes and
returns you to the Immunization Master with the new information showing.
Unsaved changes are lost,
and you are returned to the Immunization Master.
