TB Contacts

 

The Tuberculosis Contacts section allows you to enter information about the people who may have been exposed to TB through contact with the client.

 

TB Contacts Screen

 

 

The TB Contacts screen consists of three sections.  The upper section contains information about the client.  The middle section contains information about the contact interview of the TB client.  The lower section is a list of the contacts for the TB client.

 

Client Section

 

Field

Description

Site ID

(Read-Only)  Identifier and name of the site entered in the TB Master.

Source Case

(Read-Only) Identifier or ID of the source case entered in the TB Master.

SSN

(Read-Only) Client’s Social Security number.

Private Doctor

Name of the client’s private physician

Status

(Read-Only)  Client’s TB status entered in the TB Master.

Predominant Site

Defaults to predominant site selected in TB Master.  Select from a pull-down list.

Site, If Other

Defaults to Other Site entered in TB Master.

Duplicate Contacts To

List IDs or identifiers for people with the same contacts as the source case.

Reason No Contacts ID

List reasons if there are no contacts.

 

Interview Information

Date Reported

Date the TB was reported. Must be a valid date on or after the client’s birth date and on or before the current date.

Interviewer

(Required Field) Provider who conducted the interview.  Select from a pull-down list.

Interview Date

Date interview was conducted.  Must be a valid date on or after the client’s birth date and on or before the current date.

TB Clinic

Name of the TB clinic.  Select from a pull-down list.

 

Contact List

(Read-Only) ID of the contact

Contact Name

(Read-Only)  Last and first name of the contact

DOB

(Read-Only) Contact’s date of birth

Positive TST?

(Read-Only) Indication whether contact tested positive for TB (True or False)

 

 

From the TB Contacts screen, you are able to access the following buttons:

 

 

Save

 

Saves the TB Contacts information

 

Add

 

Save the TB Contacts information and brings you to a the TB Contact Information screen where you can enter a new contact

 

Change

 

Disabled until you highlight a contact row.  Takes you to the TB Contact Information screen for the selected contact.

 

Delete

 

Disabled until you highlight a contact row.  Deletes the highlighted contact from the database.

 

TB-340 Report

 

Brings you to the TB-340 report for the client.

 

Exit

 

Returns you to the TB Master without saving any changes.

 

TB Contact Information Screen

 

 

 

This screen allows you to capture specific information about each person who has come into contact with the TB client.  The screen has three sections plus a Notes section.  The uppermost section captures demographic information about the contact.  The middle section captures TB testing history and results.  The lowermost section captures the status of the TB treatment for the contact.

All fields (if entered) must be numeric.  The area code must be three digits, and the phone number must be seven digits.

SSN

Contact’s Social Security number.  Must be numeric and nine digits long.

Sex

Contact’s gender.  Select from radio buttons.

Race

Contact’s race.  Select from a pull-down list.

DOB

Contact’s date of birth.  Must be a valid date on or before the current date.

BCG

Check if contact had BCG.

Relation to Case

Free text field.  Enter relationship of contact to the TB client.

Exposure Risk

Rate the level of exposure risk.  Select from a pull-down list.

Exposure Site

Place where the contact was made.  Select from a pull-down list.

Date Contact Broken

Date contact stopped interaction with client.  Must be a valid date not before the contact’s date of birth and on or before the current date.

All fields (if entered) must be numeric.  The area code must be three digits, and the phone number must be seven digits.

Number of months recommended for the treatment regimen.  Must be numeric.

Number of months the treatment regimen was followed.  Must be numeric.

Clinic

Free text field.  Name of clinic where treatment was administered.

 

Contact Notes Section

Large text area where you can enter any pertinent information about the contact.

 

From the TB Contact Information screen, you are able to access the following buttons:

 

 

Save

 

Saves the contact information and returns you to the TB Contacts screen with the new contact appearing in the contact list at the bottom of the screen.

 

Exit

 

Returns you to the TB Contacts screen without saving any changes.

 

 

TB-340 Report

 


A Note on Printing the TB-400A, TB-400B, and TB-340 Reports:


There are checkboxes on these reports that TWICES will fill in (blacken)
for each true condition. If you see the blackened boxes on your screen
but not on the printed form, you need to change a setting in your
Internet Explorer. From the menu at the top, click on Tools,
then Internet Options. Then click on the Advanced Tab.
Scroll down until you see Printing. There will be one option called
Print Background Colors and Images. Click the checkbox next to it,
and the boxes will print correctly.

 

If the report breaks into two pages, go to File, then Page Setup in Internet

Explorer and set the Top and Bottom margins to 0.25.

 

 

 

 

This report is generated dynamically when you press the TB-340 Report button from the TB Contacts screen.  You will want to save all of your contact information before generating this report.  The Contacts Information (C) Section of the report will dynamically add as many contacts as you have entered.
