Immunizations – Add Current and Change Current

If your security level is 4 or 5, when you press the Add Current button to add immunizations for a client, you will be brought to the screen above in order to choose the site where the immunizations will be administered.  If you have a security level of 3 or below, you will be brought directly to the Add Current screen where the site will default to your clinic.

Change Current Screen

 

There are two screens for adding and changing current immunization records – Add Current and Change Current. 

The Add Current screen enables you to add up to eight shots and a PPD service for an immunization visit.  Access this screen by pressing the Add Current button on the Immunization Master screen.

The Change Current screen allows you to modify the shot records for a specific visit.  You can access the Change Current screen by highlighting the date of a current (non-history) shot on the Immunization Master and pressing the Change Current button.  This button will be disabled until you highlight a current shot date.  When you are brought to the Change Current screen, all the shots for that visit will be displayed and can be modified.  The visit information in the upper section of the screen can also be modified.

 

Following are the fields for both the Add Current and Change Current screens:

 

Visit Information (Upper Section of Screen)

Field

Description

Service Date

(Required field)  Date the immunization was given.  Date must be on or before today's date.

Site

(Required field)  TWICES clinic name.  Select from pull-down list.  Default for the field is your clinic.

Encounter Site

Location where immunization clinic is held.  Select from pull-down list.  Default for the field is Office.  

Program

Program requesting the service.  Select from pull-down list.  Default for the field is Immunizations.

Screener

Service provider who screened the client and determined the required immunizations.  On a change, if the screener has deactivated since the services were given, the name will show in gray on both the screener and provider lists.  

Provider

(Required field)  Service provider who administered the immunizations.  On a change, if the provider has deactivated since the services were given, the name will show in gray on both the screener and provider lists.  

Consenting Adult

If the client is underage, name of the person who is legally responsible for the client.

Relation to Client

If the client is underage, relationship between the Consenting Adult and the client.  Select from a pull-down list.

VFC Cat

Category for Vaccines for Children.  Defaults to VFC Category specified in Immunization Master.  Select from a pull-down list.

Reaction

Indicates if the client had a reaction to immunizations given on this service date.  Select from a pull-down list.

Comments

Free-form comment field.  

VAERS

Indicates that a VAERS (Vaccine Adverse Event Reporting System) form was completed for the client's reaction.  Select a Yes or a No radio button.

Immunization Records (Middle Section of Screen)

CPT

(Required field or automatically filled in if VACCINE is selected.)  Indicates CPT code of the vaccine given to the client. 

Description

Automatically filled in if CPT is entered.   Indicates description of vaccine given to the client.  Select from a pull-down list.

Lot No

(Required field)  Indicates lot number given to the client.  LOT NO is based on Vaccine Inventory Records.  Select from a pull-down list.  A lot number displayed in red indicates an expired lot.  A lot number displayed in orange indicates a lot with a negative quantity. 

 

On the Immunization – Change Current screen, if the lot has been de-activated since the shot was given, the lot number will be grayed out and the pull-down will be disabled.  If you need to change the shot record to an active lot, you will have to change the CPT Code to another code and then back to the original CPT code to refresh the lot number pull-down.  The de-activated lot will not appear in the refreshed list.

Mfg

Automatically displays after LOT NO is selected.

Body Site

(Required field)  Site where the immunization was given.  Select from a pull-down list.

VIS Date

(Required field or automatically filled in if CPT or Description is entered.)   Date of the Vaccine Information Sheet given to the client or the consenting adult.  This field can be overwritten.

Delete?

(Change screen only) You will see a Delete checkbox to the right of each immunization.  If you check this, the immunization will be deleted when you press Save. 

PPD Record  (Bottom Section of Screen)

PPD - Lot No

(Required field) Indicates lot number given to the client.  LOT NO is based on Vaccine Inventory Records.  Select from a pull-down list.

Mfg

Automatically displays after LOT NO is selected.

Body Site

(Required field)  Site where the immunization was given.  Select from a pull-down list.

Date Read

Date when the PPD skin test was read and evaluated.  Date must be after the service date.

Measure

Numeric results of the PPD skin test in millimeters.  Must be between 0 and 99.

Result

Result of the PPD skin test.  Select from a pull-down list.

Delete?

(Change screen only) There is a Delete checkbox to the right of each immunization.  If you check one, the immunization will be deleted when you press Save.

 

The following buttons can be accessed from Add Current and Change Current.

 

Save (Hot Key Alt + S)

 

Saves unsaved changes from all sections of the screen and returns you to the Immunization Master with the new information showing.

 

Exit (Hot Key Alt + X)

 

Unsaved changes are lost, and you are returned to the Immunization Master.
